Community Assessment Using the Nursing Process
In Week 4, the focus is on learning how to perform a community assessment with the community as the client. The nursing process can be used to assess, plan, implement, and evaluate nursing interventions in the same way that the nurse performs these functions in the acute care setting. The first step in the assessment of a community is to clearly define the population/community. That process was discussed in Week 3.
The next step in the assessment process is to gather data from which health care needs can be determined. In a community assessment, there are two types of data (existing data and new data) that are important for a complete picture of the designated community/population.
Existing Data
This type of data includes census information, vital statistics, reports available from the Department of Public Health, data from the Centers for Disease Prevention and Control (CDC), etc. The community health nurse needs to become proficient in ways to search for existing data that will support the specific population/community that he/she is working with.
