In this practicum assignment, you will complete a Health Promotion Risks and Needs assessment, select one topic of concern raised during the assessment and then develop a plan of care that includes a videotaped teaching session.

Step 1

To be completed week 3:

• Obtain a signed consent from an individual who is willing to participate in Steps 2, 4 and 5 of the practicum assignment. Submit the signed consent form into the “Consent Form” drop box by Sunday at 11:59 pm (THIS SECTION IS ALREADY COMPLETED)
  

Step 2

To be completed weeks 4-5:

a. Conduct a health promotion assessment using the “Health Promotion Assessment Worksheet” and the grading rubric as your guide.

b. Write a 3-4 page paper using APA format.

a. Summarize your assessment findings, using the same headings as the Health Promotion Assessment Worksheet

b. Identify ONE priority topic of concern with a health education need

c. Develop a nursing diagnosis based on the topic of concern

Step 3

To be completed by week 6:

a. Conduct a review of literature to find supporting evidence for the effectiveness of an education intervention aimed at the topic of concern.

Find evidence of at least FOUR education strategies or trainings that were published in the past 5 years that addressed the topic of concern in a similar

population to the individual interviewed in step 2 (ie. by sex, culture, health condition).

Use at least three scholarly sources (that is, peer-reviewed journal articles or websites written by a group that has expertise and strong credentials in the field of interest).

b. Develop an evidence table for the review using the Health Education Needs Evidence Table and the grading rubric

a. Identify and describe a teaching strategy based on evidence

c. Upload your evidence table into the assignment drop box by Sunday at 11:59 pm MST week 6

Step 4

To be completed by week 7:

a. Develop and practice an 8-10 minute educational session based on the teaching strategy identified in step 3. Present the education to the individual in a videotaped session.

c. Upload your video to YouTube a. Copy the URL link to your unlisted YouTube video upload.

b. FORMAT: MP3 or WAV file (or any format consistent with YouTube)

d. Paste the You Tube link into a Word document and submit into the drop box by Sunday at 11:59 pm week 7

Step 5

To be completed by week 8:

a. Select and administer an evaluation tool that is appropriate for the educational session delivered in week 7.

a. Examples: return demonstration, pre-test-post-test, satisfaction survey

b. Write a 2-3 page paper in APA format outlining:

a. The evaluation method selected and rationale for the choice of method.

Cite sources by APA format. If a tool was used, please include it with the paper as an attachment

b. The results of the evaluation

c. Self-reflection regarding effectiveness of the teaching experiencec. Use the week 8 assignment rubric for grading requirements
